We want to help you get the most out of your Southwest Medical primary care visit. Here are some steps to help streamline the process and get you the care you need.
Choose your primary care provider
It’s important to have a primary care provider that you trust and is convenient to access. Use the
Find a Provider
tool to find your new primary care provider. You can change your primary care physician at any
time. However, we hope that you’ll build a long-term relationship with your doctor and your team —
focused on your good health.
Find a providerBefore your appointment
- Print and fill out the new patient packet to bring with you. This will save time at check-in.
- Print and fill out the checklist to ensure you have everything you need for your appointment.
- Plan to arrive 15 minutes ahead of your appointment time.
- Bring your prescription bottles or a list of your medications, including herbs and vitamins. This list should include the name of the medication, dosage and the provider who prescribed it to you.
- Bring any previous, relevant medical records you may have.
- Follow any pre-appointment instructions you may have been given (fasting, liquids, medicines, etc.).
- Make sure you have your health insurance ID card and picture ID.
What to expect at your appointment
- Check in at the front desk and confirm your address, phone number and insurance information. You’ll pay your copay before your appointment.
- You’ll complete a general medical history form (first-time visits or yearly for some services).
- The nursing staff will check your vitals.
- You will meet your provider and discuss any health concerns or questions. You’ll also talk about current prescriptions and any necessary follow-up or referrals.
- Your provider will review and update your medical record electronically during the exam. He or she will help you with next steps for any preventive exams or tests that are due.
- A member from the nursing staff will provide you with your check-out paperwork.
Some things to remember
- Please call the scheduling line to cancel an appointment if you can’t make it.
- Be honest with your provider. Communicate any health concerns you might have. This will allow your provider to give you the right care based on your needs.
