Common questions about Southwest Medical.

If you wish to change the doctor you’re empaneled to, please call the member services number on the back of your insurance card.
Call our customer care team at 1-702-877-5199, Monday–Friday: 7 a.m. to 8 p.m., Saturday–Sunday: 7 a.m. to 5 p.m.

  • For primary care appointments, print out the new patient packet. Fill out the information and bring it with you to save time.
  • Bring your prescription bottles or a list of your medications, including herbs and vitamins. This list should include the name of the medication, dosage and the provider who prescribed it to you
  • Bring any previous, relevant medical records you may have.
  • Health insurance ID card and picture ID (for example, your driver’s license).
If you can’t make your appointment, please call our customer care team at 702-877-5199 at least 24 hours prior to your appointment. This will help us use that time for another patient.
First, you’ll want to schedule an appointment with your primary care provider to discuss and concerns or conditions you have. Based on your visit with your PCP, if a referral to a specialist is needed, they will process that for you and give you next steps.

Southwest Medical has partnered with FollowMyHealth to provide you access to your electronic medical records. FollowMyHealth also allows you to email your provider and renew prescriptions.
